Form 4506-T (Request for Transcript of Tax Return) is faxed to one of three RAIVS units: Kansas City 855-821-0094, Austin 855-587-9604 or Ogden 855-298-1145. The numbers are stable but the IRS reshuffles which states map to which unit — always confirm on the current where-to-file page before sending.
