Form 4506-T (Request for Transcript of Tax Return) is faxed to one of three RAIVS units — Kansas City 855-821-0094, Austin 855-587-9604 or Ogden 855-298-1145 — but the IRS keeps TWO different state charts: one for individual transcripts (Form 1040 series, W-2, 1099) and one for all other transcripts. The same state can route differently: a Texas 1040 transcript goes to Austin, a Texas business transcript to Ogden.
